You may submit form and $15 payment made to GSCO to the day camp director.

Health Forms:
Please complete the following forms which you can fill in online.  We must receive by June 1, 2015

Adult volunteer health form in English       Spanish

Camper, PAI, PA and Tagalong health form in English      Spanish

Medication Permission form - we need this form for permission to give anything like tylenol, benedryl, etc.  Please also list their sunscreen and bug spray.  Please submit this with the health form.  You can add to it at camp if needed.  Due no later than the first day of camp - June 15, 2015

Waiver & Release - Due no later than June 1, 2015  PDF version

Photo Release form - Due no later than June 1, 2015   PDF version

Carpool Authorization - Due no later than June 1, 2015    PDF version

Cookie Credits form and Information

Please complete this and have the check mailed to our PO Box if you are needing to use cookie credits as part of the full payment due.  Please provide a copy of the form by email or mail so we know how much to expect.  Our address is on the "Contact Us" page.

If you can pay for camp in full by the date the full payment is due, please complete the form and you can have the reimbursement check sent back to you.
